  •   Incorporate consumer choice. assess capacity and unmet/unspoken needs
  •   Select and use validated assessment tools
  •   Formulate care plans that incorporate safety, risk assessment and management
  •   Develop consumer-focused goals
  •   Assess and plan for clinical and health-related needs
  •   Evaluate and monitor care plans
