Care/Case Management Practice

What does the course consist of?

This six- module course has been aligned with units of competency for the National Skills set -Case Management Skills- and consists of theory and practical application of learning in the workplace. It includes pre reading, case studies and extensive resources.

What is covered in the modules?

Modules include assessment, care planning, goal setting, case conferencing, documentation; monitoring of consumer changes. The training includes competency testing via an interactive learning style. The training has been modified for the home care environment to ensure practical examples of how learning can be translated into practice.

What evidence will I get as evidence of attendance ?

All attendees will receive a Certificate of Attainment upon completion with evidence of Continuing Professional Development hours.

Who is the course suitable for ?

The course is suitable for those who are new to care management roles and staff who want to refresh their skills.

It is of particular use for providers who want to promote consistency of practice within their teams and ensure staff are aware of how important their roles are in meeting quality standards; evidence consumer engagement; understanding duty of care and to better manage risks.
