Support at Home transition grant opens

A grant has opened for providers operating in thin markets to assist with the transition to Support at Home.

Home Care Package and Short-Term Restorative Care Program providers servicing regional and remote locations or delivering specialised services that operate in thin markets are eligible to apply for a one-off year of funding in 2025-26 to assist transitioning to the Support at Home program.

The total amount available is $297,400,000.

Specifically, the grant is targeting those who currently deliver services to care recipients in Modified Monash areas 3-7 or currently hold, or have applied for, a specialisation verification for aged care services. This excludes specialisation verification for people who live in rural or remote areas.

Potential grantees are reminded the Federal election may delay or cancel the grant process.

For an application to be considered, the applicant must be one of the following entity types:

  • Indigenous corporation (registered under the Corporations (Aboriginal and /or Torres Strait Islander) Act 2006)
  • company
  • corporate Commonwealth entity
  • non-corporate Commonwealth entity
  • non-corporate Commonwealth statutory authority
  • corporate state or territory entity
  • non-corporate state or territory entity
  • non-corporate state or territory statutory authority
  • local government
  • cooperative
  • incorporated association
  • statutory entity
  • partnership, or
  • unincorporated association.

If applying as a trustee on behalf of a trust, the trustee must have an eligible entity type.

Other eligibility criteria includes all mandatory attachments being provided, including a correctly completed thin markets care recipient template.

If the organisation or a project partner is included on the National Redress Scheme’s website on the institutions that have not joined or signified their intent to join the scheme list the application will not be accepted.

Applications will also not be accepted if the organisation or project partner is included on the Workplace Gender Equality Agency website on the non-compliant organisations list.
