New guide for Support at Home transition

In the lead up to the commencement of Support at Home, the department has published the necessary steps providers need to take.

The Department of Health and Aged Care has released a new resource to assist providers with the transition to the new Support at Home program from 1 July.

The Support at Home Program Provider Transition Guide outlines the steps for Home Care Packages and Short-Term Restorative Care providers to be operationally ready by 1 July when the new SaH program replaces the HCP and STRC programme.

The Support at Home program provider transition guide covers:

  • understanding the impacts of the SaH program
  • preparing for transition
  • delivering services
  • receiving payments
  • facilitating change in service if needed.

The guide also includes links to several resources and tools to support providers during the transition and outlines the anticipated release timeframe for further information.

Periodic updates will be made to the guide as additional support tools and resources are released, and the department recommends providers regularly check their website for further information.
