Medication management platform rolled out across SACARE sites

SA disability provider SACARE is rolling out an electronic medication management tool across its five residential sites.

Following a successful trial period, South Australian disability provider SACARE is rolling out an electronic medication management tool across its five residential facilities and community-based clients.

While piloted at SACARE’s Kingswood site in Adelaide, the Medi-Map platform completely eliminated medication incidents.

“The greatest outcome of the pilot is the reduction in errors,” said SACARE general manager of clinical therapy services Klade Thomas. “By reducing errors, we reduce the risk of harm to people in our care.”

Medication mismanagement, misuse or misadventure is a major issue in Australia, accounting for 250,000 hospital admissions a year at a cost of more than $1 billion –according to the Pharmaceutical Society of Australia.

Specifically designed for the healthcare sector, the technology replaces SACARE’s old-school method of medication management, said Ms Thomas. “Clients previously had multiple hand-written charts that included medication information, but these can be impossible to read and understand, raising a safety risk.”

In contrast, Medi-Map provides clear, easy-to-read information, said Ms Thomas. “It has built-in reporting mechanisms allowing for greater oversight and clarity of the clients’ medication information – it can also be used offline, which is integral if there is a power outage,” she added.
